A photocopy of your teaching license, if you have a teaching license. An employment verification form on official school letterhead signed by your principal or other administrator verifying your employment. This form must include the name(s) of the school(s) where you have taught as a full-time teacher for at least three years and include the dates of employment.?Please note: the teaching experience must be in the state in which you hold the license.
